Be sure to include the nationality of all dependents on the letter. Include the date of birth for all children. Children must be at least 18 years of age to travel alone with this letter. Please remember to keep the original letter with the original signature throughout the period of sign-up and travel.

*For Army personnel a signature from the unit First Sergeant or Sergeant Major may be accepted if the commander is deployed. The signature block must state First Sergeant or Sergeant Major and must have their signature. The signature of an acting First Sergeant or rear detachment commander below the rank of First Sergeant will not be accepted for the purpose of the command sponsor letter or category upgrade letter
